Who is eligible to enroll as a participant?
Any person who meets the medical necessity criteria for Adult Day Health Care services and eligibility criteria for CBAS; this includes those with a medical, mental or cognitive condition that requires monitoring, therapy or anyone at risk for institutionalization (ER visits, hospitalization, skilled nursing facilities). Conditions such as, but not limited to High Blood Pressure, Diabetes, Heart Disease/CHF, Arthritis, COPD, Renal Failure, CVA/Stroke, Dementia, and Alzheimer's Disease. Joint pain and stiffness, Hearing/Vision impairment, Wheelchair bound, speech and swallowing difficulties.
Who pays for this?
CBAS services are available at no cost to Eligible Medi-Cal Managed Care Members of Kaiser, Care 1st, Health Net, Anthem Blue Cross, LA Care and to qualified members of SCAN Health Plan and Veterans Administration.
